출생시 체중은 신생아사망률에 영향을 주는 여러 가지 요인들 중에서 가장 중요한 요인일
뿐만 아니라, 신생아와 영아 사망률 및 유아기의 질병 이환율과도 유의한 관련성을 보이고
있다. 출생 체중에 따른 신생아 사망률을 보면 1,500gm미만의 경우는 45∼55%, 1,500∼
2,500gm은 3∼5%로 2,500gm 이상의 1% 미만에 비하여 매우 높다. 또한 주산기 사망률 및
영아 사망률의 70% 이상이 저출생 체중아와 관계있다는 보고가 있는 반면, 26.4%라는 보고
도 있다. 출생시 체중이 2,500gm 미만일 때 저출생 체중아라고 하는데 이의 약 2/3는 재태
기간 37주 미만의 미숙아이고, 1/3은 산모, 태반 및 태아에서 여러 가지 원인으로 재태기간
에 비해 출생 체중이 작은 부당 경량아이다. 저출생 체중아는 전체 출생아의 10%미만이지
만 생후 첫주에 발생하는 조기 신생아 사망의 50% 이상을 차지하고, 생존한다 하여도 신체
적, 정신 신경학적 장애를 남기는 경우가 많고 호흡기, 순환기, 위장관 및 비뇨기계 등에 많
은 문제를 일으키며 체온 조절이 불리하고, 광물질, 비타민, 면역체 등의 부족으로 구루병,
빈혈, 감염 등의 위험도 높다.
이와 같이 신생아 및 영아의 건강과 사망에 중요한 원인인 저출생 체중아와 관련된 요인
으로는 산모 자신, 태아 및 태반의 문제 등으로 대별할 수 있다. 이 중 산모측 요인에 대한
연구에서는 산모 자신의 신체적 조건, 생활 습관, 병력 유무 및 임신 조건 등이, 그리고 태
아측 요인에 대해서는 태아 곤란, 다태아, 선천성 기형이나 감염 등이 주 관심이 되고 있다.

To investigate the incidence of low birth weight infants and their mortality and its
related factors, a study was conducted for 2,179 newborns who were born at three
hospitals in Kwangju from January 1 to December 31, 1994. Data were collected by
telephone interview with mothers and from hospital records during the period from
December 1995 to January 1996.
The incidence of low birth weight infant(<2,500gm) and very low birth weight
infant(< 1,500gm) were 7.4% and 1.4% respectively. Infant mortality rate of low birth weight babies was 23.6% and significantly higher than that of normal birth weight infants(0.48%).
Birth weight was related to maternal age, educational level and illness during pregnancy, and birth order and congenital anormaly of file newborn. Especially, the factors related to low birth weight were illness during pregnancy, placenta related illness, and female birth.
Infant mortality rate was significantly related to c-section delivery and illness during
pregnancy.
